HOW I STORE YOUR PERSONAL INFORMATION

• Paper: Written notes will be securely filed in a locked filing cabinet.

•  Laptops / Personal Computers: Written notes may be kept on a secured personal computer.  The device will require passwords to access both the main account and to access your individual information.  The electronic device will be checked regularly for viruses and I will take every reasonable precaution to protect your personal information.

• Mobile Phone: I may store your contact telephone number for the duration of your treatment along with any text messages we may exchange. 

• Email: Your email address and email correspondence will be stored securely in our email accounts (iCloud & Wix web mail account).

• Website: None of your personal information is stored on my website, other than to momentarily collect & send it to our email accounts for the purposes of initial contact.

HOW I MAY PROCESS/SHARE YOUR PERSONAL INFORMATION.

Your contact details and personal information will never be shared with outside parties. 

Personal contact details are only ever used so I can contact you regarding session bookings, feedback, payment or refund details, notify you of changes in services, events and promotions that relate to your interests and support. 

BREAKING CONFIDENTIALITY  

If I believe yours or another person (or group of people’s) safety is at significant risk, I may contact the suitable authority without your permission.  This includes your doctor, the emergency services and the mental health crisis team.

We are also required by law to pass on information relating to acts of terrorism and on going abuse of a minor or vulnerable adult.  We will at all times attempt to gain your permission before breaking confidentiality in these instances however we retain the right to do so without your consent.

ERASING YOUR INFORMATION.

All the information I hold about you will be held for a maximum of 7 years after your final session with me (or from the last point of contact if you choose not to go ahead with treatment).  All paperwork will be shredded. All digital information deleted. It is a legal requirement for therapists to hold personal records for a specified time to cover legal and insurance policy terms and conditions.   

YOUR RIGHTS UNDER GDPR

To see the information I hold about you.

To rectify any inaccurate or incomplete personal information.

To withdraw consent to me using your personal information.

To request that some or all of your personal information be erased (though we can decline whilst the information is needed for us to practice lawfully & competently).

I ALSO NEED YOUR PERMISSION & PROOF OF YOUR UNDERSTANDING THAT I WILL HOLD YOUR PERSONAL DATA (EMAIL, PHONE, ADDRESS, DOCTOR INFO, HEALING SESSION RECORDS). 

 

THIS DATA IS CONFIDENTIAL & NEVER SHOWN, DISCUSSED OR SOLD ON TO A THIRD PARTY. 

 

BY LAW I MUST HOLD ON TO ALL PERSONAL RECORDS FOR SEVEN YEARS AFTER OUR LAST APPOINTMENT. AFTER WHICH ALL WILL BE DELETED OR SHREDDED.
